	Gift Donation

	A gift to honor or in memory of a loved one will help support research for pancreatic cancer. One hundred percent (100%) of your gift donation to PCAF will help fund pancreatic cancer research. Your gift will make a difference!

	Gift Card
	The family or individual will receive a PCAF Gift card notifying the family of your kind donation in honor of their loved one. The amount of your gift will not be disclosed.

	Confirmation
	Your request will be acknowledged by email or mail when we process your information.

	Your Donation
	The minimum amount is $25. Your tax-deductible gift will help fund research for pancreatic cancer. PCAF is a tax-exempt nonprofit organization under IRS Code Section 501 (c) 3.

	How to Donate
	Please print Gift Donation form (see "Print Form"), and mail your completed form with a check made payable to: " PCAF " or Pancreatic Cancer Awareness Foundation Memorial Gift PO Box 2111 Sarasota, FL 34230-2111
